Lecture "Clinical anatomy and biomechanics of the upper cervical and subaxial levels of the cervical spine. Stability of craniovertebral transition, radiological criteria for diagnostics of craniovertebral injuries. Stability of the subaxial level of the cervical spine, diagnostic criteria for unstable injuries. The lecture will cover the main anatomical and biomechanical features of the two sections of the cervical spine.
Lecturer: Alexey Lastevsky, deputy director for medical work, head of the trauma and orthopedic department No. 1 at the Ya.L. Tsivyan Research Institute of Traumatology and Orthopedics of the Ministry of Health of Russia, candidate of medical sciences, traumatologist and orthopedist, neurosurgeon.
09:50-10:35
Lecture "Modern classifications of injuries of the upper neck and subaxial spine. Approaches to determining treatment strategy. Surgical treatment tactics". This lecture will focus on treatment strategies for injuries of the cervical spine from the standpoint of modern classifications, and the principles of determining the indications for conservative and surgical treatment of injuries.
Lecturer:
Alexey Lastevsky - deputy director for therapeutic work, head of the trauma and orthopedic department No. 1 of the Ya.L. Tsivyan Research Institute of Traumatology and Orthopedics of the Ministry of Health of Russia, candidate of medical sciences, traumatologist and orthopedist, neurosurgeon.
10:35-11:20
Lecture "Principles of instrumental fixation of the upper and subaxial spine for its injuries, post-traumatic deformities and diseases". The lecture will cover modern principles of anterior and posterior surgical instrumentation of the craniovertebral transition and subaxial spine for unstable injuries, modern requirements for fixation, and surgical techniques.

Coffee break
11:35-13:30
Practical exercises:
1) Occipitospondylodesis.
2) Fixation of C1-C2 vertebrae according to Harms.
3) Posterior instrumentation at the level of C2-Th1 by lateral masses, translaminar and transpedicular cervical fixation.
The purpose of the hands-on session is to practice skills in posterior surgical instrumentation at the level of the craniovertebral transition and subaxial cervical spine.
Moderators: Lastevsky A.D., Aslanov R.A., Aleinik A.Y.
13:30-14:00
Lunch

Lecture: Combat surgical trauma of the neck. Penetrating wounds of the cervical spine. Mine blast trauma, gunshot trauma. Classification, principles of treatment. Peculiarities of care.
Lecturer: Aslanov Ramis Aslanovich, orthopedic traumatologist, head of the spine surgery department of State Medical Center named after ac. N.N. Burdenko.
14:45-15:30
Practical training: "Surgical reconstruction of the anterior support column at the C2-Th1 level using anterior cervical plates and anterior support implants.
The purpose of the practical session is to practice skills in anterior surgical instrumental fixation of cervical spinal-motor segments for traumatic instability.
Moderators: Lastevsky A.D., Aslanov R.A., Aleinik A.Y.
15:30-16-00
Discussion, discussion of clinical cases

17:00
Dinner
09:00−11:30
Clinical Case №1. Anterior bi- and monosegmental spondylodesis at the subaxial cervical level for an unstable injury (operating room).
Operating physician: Lastevsky Alexey Dmitrievich - MD, PhD, orthopedic traumatologist, neurosurgeon, head of the trauma and orthopedic department No. 1 of the Y.L. Tsivyan Research Institute of Trauma Surgery of the Russian Ministry of Health.
Broadcast from the operating room to the audience.
Present in the operating room - 5 specialists.
The goal is to demonstrate the clinical application of interbody cages and anterior cervical plates, in the reconstruction of the anterior support column of the spine in real operating room conditions.
Expected results: Participants in the practical part of the course will learn about the optimal placement of the patient on the operating table during reconstructive interventions in the anterior cervical spine, the principles of closed and open reduction of cervical spinal dislocations, the principles of intraoperative visualization of operated spinal segments, methods of preparing the interbody space for cage implants, prevention of possible intraoperative complications associated with implant placement.

Clinical Case №2. Posterior instrumental stabilization of the spine at the level of the craniovertebral junction (operating room).
Operating physician: Alexey Lastevsky, MD, PhD, orthopedic traumatologist, neurosurgeon, head of the trauma and orthopedic department No. 1 of the Ya.L. Tsivyan Research Institute of Trauma Surgery of the Russian Ministry of Health.
Broadcast from the operating room to the audience.
Present in the operating room - 5 specialists.
The goal is to demonstrate patient positioning, surgical access, and anatomical landmarks during insertion of screws into the posterior structures of vertebrae at the level of the occipital bone, C1, C2, and C3-Th1 vertebrae.
Expected results:
Participants in the practical part of the course will gain knowledge of the principles of patient positioning for posterior cervical surgery, the principles of choosing points of insertion of fixation elements into the bone, methods of intraoperative reduction, and correction of deformity.
